We need you to bring:
a) Two officially issued letters that clearly show your name and registering address which must be dated within the last three months (such as council tax, utility bill, bank statement, mortgage statement, letters from DWP or Home Office) We do not accept tenancy agreements, medical cards or driving licences as proof of residence.
However, if you are unable to prove your address, for example if you are homeless or in temporary accommodation, we will still be able to register you.
b) A officially issued photo ID (such as driving licence, passport).
c) If you have had a GP in the country before we need the name and address of your previous GP and your NHS number.
We can register you anytime, however if possible, please avoid busy times. This will help us reduce your waiting time.
As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and we are unable to process your application without this information.
Medical treatment is available from the date of registration. It can take up to ten working days to process your application form, so you are advised to register in advance of a medical need. Please contact reception for further information.
